The Consultation
We pride ourselves on focusing on quality over quantity, and we can do that thanks to our seamless admission period, which starts with a consultation. Your hospice journey with us will begin with a thoughtful consultation.
We’ll get to know you and your thoughts, values, and concerns during this consultation. You’ll tell us your wishes and goals for hospice care. We’ll listen to your concerns and answer all your questions so you can make an informed decision.
Creating a Custom Plan
Your answers during the consultation are used to construct a personalized care plan customized to your unique emotional, physical, and spiritual needs. At Homage Hospice, our benefits extend far beyond traditional healthcare. That means we take a holistic approach to ensuring that the physical symptoms are managed and each individual’s emotional, social, and spiritual needs are addressed. To do this, you’ll also have visits from our team of social workers and spiritual counselors so that your emotional, psychosocial and spiritual assessments are added to your care plan.
During this process, our nursing staff and physicians will collaborate with your primary healthcare physician to create a care plan that ensures a peaceful transition with respect and dignity.

During the admission process, you can expect a fair amount of paperwork as these documents are used to gather crucial information about our patients and create an appropriate care plan. But the good news is that you’re never alone. Our compassionate team will be there 24 hours a day to provide assistance and answer any questions you or your family may have.
The First Couple of Days
Like your first day at school or a new job, your day will be quite busy. This is when documents are signed, and medication and medical equipment orders are placed. Other equipment like hospital beds and commodes are being installed. Medical supplies and personal hygiene supplies will be delivered.
There’ll also be a number of visitors stopping by so you or your loved one can meet team members who will very quickly become familiar faces.
