Hospice social workers play a crucial role in helping patients and their families navigate the challenges of the end-of-life journey.

• Provide emotional support. In hospice care, the emotional needs of the patient and their family are important, and social workers listen to patients’ problems, concerns, and thoughts to help them cope with their unique situations. Social workers can help the patients and their families process the complex emotions that arise during the end of life, including fear, grief, anxiety, and loss.
• Assist with care planning. In addition to providing emotional support, hospice social workers can help patients and their families create meaningful end-of-life plans that reflect their beliefs, wishes, and goals. They can also help patients and families complete important paperwork for medical insurance and offer guidance on making major medical decisions, such as creating and signing forms for advance directives or do-not-resuscitate (DNR) orders. This level of care and guidance doesn’t stop after death. Hospice social workers assist in filling out paperwork after the death of a patient.
• Connect families with support services. Hospice social workers are experts in navigating the healthcare system and can help by connecting families with resources they didn’t know were available. They can also help identify what services patients and families might need, such as financial assistance programs and visiting physician services. The end-of-life journey can be overwhelming, and families may not know how to deal with complex emotions in a healthy manner. Social workers can help connect families to counseling services and emotional support groups.
